Abstract:

The local control rate of rectal cancer has increased with multimodal treatment consisting of chemotherapy, surgery, and radiation therapy; however, overall survival and the risk of metastatic relapses have remained unchanged. Our study's objective was to examine the prognostic determinants of overall survival, disease-free survival, and locoregional and distant control. This study looked back at patients who received radiochemotherapy for locally advanced non-metastatic rectal cancer. We gathered sixty-six patients. In 46 patients, the tumor was located in the lower rectum. In 20 cases, it was categorized as N0, and in 49 cases, as T3. In 47 of the cases, lieberkühnian adenocarcinoma was the most common histological type. Six patients had surgery right away, and 24 patients who had neoadjuvant treatment had a T3 tumor discovered. Twenty-five patients had node invasion. Six patients (13.3%) who underwent surgery following neoadjuvant treatment had a full histological response. Tumor perforation, peri-neural engorgement, and recurrence were factors linked to a lower OS. The occurrence of lymphovascular emboli, invaded circumferential borders, and adjuvant CT delays longer than 8 weeks all had a substantial impact on event-free survival. Metastatic relapse was linked to the presence of peri-neural engorgement and positive adenopathy on histological inspection of the surgical specimen. A loco-regional relapse was caused by an infected circumferential edge.
